  WHO WE ARE

Kroniker is both an Observatory and a Community of Practice focused on innovation in the field of quality of care and the management of chronic conditions. Our main objective is to help health and social care organizations transform their systems towards improved care for chronic conditions. The center is in Bilbao, Spain.

As an Observatory, we are continuously searching for innovation and resources worldwide and drawing from new ideas, lessons learnt, case studies and scientific evidence. We actively track new improvements  at both the  policy level  and

 

the clinical level. Although we track the performance and evidence of all types of models, the expertise in Kroniker is on models of disease management wich are integrated and mainstreamed into the existing health care systems. .

As a Community of practice, we facilitate knowledge and practice sharing between clinicians, researchers, managers and policy-makers involved in projects and initiatives regarding the prevention and management of chronic diseases and organization. The Community of Practice is actively fostering the creation of a worldwide network of organizations.


Kroniker is an independent organization. The core funding comes from the training and development activities in quality and chronic care. External funding comes from both public and the private organizations particularly. The principles for accepting commercial support based on those used by UN organizations and are explicited in the document “Principles for Commercial Support”.

Dr. Rafael Bengoa is the Director of Kroniker. Isabel Durán and Amaia Gergoff are the Project Managers. Ander Rojo is the Communication Officer. We operate in Spanish, English and French. Our activities are also supported by a network of researchers and collaborators in Spain and worldwide.
